4. Community Improvement Analyst
Position Description:
The Community Improvement Analyst will be focused on aligning communities’ priorities and designing improvement projects based on state and local programs using CCMCN’s data and technology systems. The purpose is to help communities create improvements in local and state priorities, with an emphasis on health equity and decreasing disparities. Each improvement project will be designed with a community-centered approach that resonates with patients and the local service providers while being applicable for any community. . The Community Improvement Analyst will design calculations to measure activities and scorecards for an improvement-based payment model. The role will build strong relationships with community partner organizations to facilitate user and patient focus groups.
Essential Functions:
- Assisting in the design of CCMCN’s Improvement Project strategy
- Creating and maintaining CCMCN’s Improvement Project Library
- Expert understanding in state and local program initiatives; creating interoperability and alignment between programs
- Design, document, and manage Improvement Project Performance Measurement System
- Guiding visual leaderboard development and incentive payments to organizations
- Facilitate user and patient focus groups throughout the community to receive feedback from current users and patients
- Educate and enroll organizations in Improvement Projects; work directly with CCMCN’s product leads throughout implementation to ensure the highest level of satisfaction
- Run Improvement Project sprints in each community or statewide whenever possible
- Develop and maintain network maps connecting community priorities and state programs to Improvement Projects
- Ability to take a leadership role within community meetings and represent the program
- Manage reporting projects that pertain to community improvements to ensure they meet community and contractual timelines
